Sports Physicals

Participation in High School Sports


Sports 2023 - 2024 

  • For Fall Sports, physical must be done after: 07/14/22

  • For Winter Sports, physical must be done after: 10/30/22

  • For Spring Sports, physical must be done after:02/16/23


If you intend to participate in an interscholastic sport, and have not submitted an appropriately dated physical exam form to the nurse, you must submit a completed physical assessment form from a physician in order to be approved to play. Please complete the State of CT Health Assessment Form (Blue Health Form). Physical examinations must have been done within thirteen (13) months of the start of the playing season. School sports physicals are done twice during the summer in August and one day each prior to winter and spring sports approximately 1-2 weeks before the start of practice. Sports physicals do not take the place of the mandated 10th grade physical.


It is your responsibility to turn in the blue physical form at least one (1) week prior to the first day of practice. Please bring the completed blue form to the nurse's office. DO NOT TURN IT INTO THE COACH.


School Sports Physicals: If you plan to have a school sport physical during the summer, come on one of the designated days with the blue form and the Permission for Sports Physical form filled out including a parent's signature. School physicals cannot be performed without written parental permission.



All completed health assessment forms must be submitted to the nurse's office at least one (1) week before the first day of practice OR YOU MAY NOT BE APPROVED TO TRY OUT, PRACTICE OR PLAY.
